Our website was created to make it easier for Visitors to learn about the services, programs and products provided by AmeriHealth and its subsidiaries. If you are currently a member, provider, broker or group who has AmeriHealth coverage or does business with AmeriHealth, you may also choose to register for one of our portals, which will allow you to access information about certain benefits or claims information. As with all online resources, we appreciate that Visitors to our websites are concerned about the privacy of personal information. This Privacy Policy discloses AmeriHealth’s policy for collecting, using and disseminating information through its website.
1. Collection of Personal Information from Site Visitors.
When you visit www.amerihealthmedicare.com, or any one of our websites, AmeriHealth may collect and/or track: (1) your home server domain names, your email address, your type of computer, the files downloaded, the search engine used, operating system, and type of Web browser; (2) the email addresses of Visitors that communicate with AmeriHealth via email; (3) information knowingly provided by the Visitor in online forms, surveys, email, contest entries, and other online submissions (including demographic and personal profile data), and (4) aggregate information on pages accessed by a Visitor.
To better serve you, we use “cookies” to customize your browsing experience. Cookies are simple text files stored by your browser and they provide a method of distinguishing among Visitors to our sites. Personally identifiable information, such as names, postal addresses, email addresses, phone numbers, and other personal information which Visitors voluntarily provide to AmeriHealth may be added to our databases and used for future communications regarding site updates, new products and services, the confirmation of transactions through the portals, upcoming events and for other purposes in accordance with State and Federal law.

Unless otherwise authorized by a Visitor, AmeriHealth will not sell, rent, license or disclose personally identifiable information to unaffiliated third parties.
AmeriHealth will not release personally identifiable information to any party unless required to do so by government authorities or legal process in accordance with applicable law.

AmeriHealth occasionally provides links to other entities on its sites for your convenience in locating health-related information, services and products. Accordingly, a link on any of our sites does not constitute endorsement of the content, viewpoint, policies, products or services provided or advertised on that site. Once you link to a site not maintained by AmeriHealth, you are subject to the terms and conditions of that site, including, but not limited to its privacy policy.
Certain products, services or treatments referred to on other sites that have a link on our site, may not be covered under certain plans offered by AmeriHealth. Members should refer to their benefit contract for complete details of the terms, limitations and exclusions of coverage. In addition, certain providers may not be network providers. Please refer to our provider directory or call AmeriHealth to make certain the professional listed is a network provider.
5. Security Measures
AmeriHealth has implemented numerous security features to prevent the unauthorized release of or access to personal information. For example, all AmeriHealth employees are required to certify their understanding that personal information is considered confidential, that it is important to safeguard personal information, and that AmeriHealth will take appropriate action against any employee who fails to acknowledge these facts or adhere to the requisite standards of conduct.
The AmeriHealth portals use 128 bit encryption to help keep information secure. You will not be able to access certain pages of our site unless your computer has 128-bit encryption. You should also be aware that despite these and other security measures implemented by AmeriHealth, no security process is completely foolproof. We will continually assess our security procedures and modify them to protect your information and we ask that our Visitors take the appropriate measures to protect their information.
If you register for one of our portals, you will have the opportunity to receive or create a password to access or submit personal information. Visitors who register to use a portal should not divulge their password to anyone and AmeriHealth will never ask for your password in an unsolicited phone call or email. When you are finished with applications that are password protected, you should exit the page.
You should be aware that the confidentiality of any communication or material transmitted to/from AmeriHealth via this site or email cannot be guaranteed. AmeriHealth is not responsible for the security of information transmitted via the Internet. 6. Opt-Out Right

(This does not apply to collection of the type of Web browser of visitors to AmeriHealth’s site.)
7. Access to and Ability to Correct Personal Data
Upon request via regular mail or email, AmeriHealth will provide to Visitors a summary of any personally identifiable information retained by AmeriHealth regarding the Visitor. You will also be able to access personal information retained by AmeriHealth through the Member Portal. Members may seek to modify, correct, change or update information that AmeriHealth has collected by contacting us via regular mail or email at the addresses above. Some Visitors may also be able to update their information via the Member Portal.
8. Collection of Personal Data from Children
To the extent that AmeriHealth is able to determine the age of Visitors by their submissions to or communications with AmeriHealth’s site, AmeriHealth will not knowingly (a) collect or post information from individuals under the age of thirteen (”children”) without their parent’s or guardian’s consent, or (b) provide any personally identifying information collected from children, regardless of its source, to any third party for any purpose whatever.
If registration is required for any services on the AmeriHealth Site, and a Visitor submits information to AmeriHealth’s website through such a registration process that indicates that such Visitor is a child, the child’s parent or guardian will be alerted to the child’s registration and will be provided instructions on how the parent or guardian can delete the child’s registration, if so desired. No information collected through AmeriHealth site from Visitors who identify themselves as children will knowingly be used for any direct marketing or promotional purposes. If there are no age restrictions on a contest which requires registration, and if a child wins, notification and prizes will be sent to the parent(s) or guardian(s) identified in the initial registration process.
9. AmeriHealth’s Right to Contact User
AmeriHealth reserves the right to contact site Visitors regarding account status and changes to the member handbook, the privacy policy, or any other policies or agreements relevant to those who visit the site.
10. AmeriHealth’s Right to Change Privacy Policy
AmeriHealth reserves the right to change this Privacy Policy at any time by notifying Visitors of the existence and location of the new or revised Privacy Policy.
You must continue to pay your Medicare Part B premium.
The Medicare Contract is renewed annually, and the availability of coverage beyond the end of the current year is not guaranteed.
The benefit information provided herein is a brief summary, not a comprehensive description of benefits. For more information contact the plan. Benefits, formulary, pharmacy network, premium and/or copayments/coinsurance may change January 1, 2013. Members may enroll in the plan during specific times of the year. Please reference the Evidence of Coverage for information on premiums, cost-sharing, out-of-network coverage, rights and responsibilities upon disenrollment and any applicable conditions associated with using the plan benefits, as well as limitations, copayments, and restrictions.
Members must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances, and quantity limitations and restrictions may apply.
You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for extra help, call:
- 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048, 24 hours a day/7 days a week;
- The Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call, 1-800-325-0778; or
- Your State Medicaid Office.
People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If you qualify, Medicare could pay for up to seventy-five (75) percent or more of your drug costs including monthly prescription drug premiums, annual deductibles, and co-insurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don't even know it. For more information about this Extra Help, contact your local Social Security office or call 1-800-MEDICARE (1-800-633-4227), 24 hours per day, 7 days per week. TTY users should call 1-877-486-2048.
You must use plan providers except in emergency or urgent care situations or for out-of-are renal dialysis or other services. If you obtain routine care from out-of-network providers neither Medicare nor AmeriHealth 65 NJ HMO will be responsible for the costs.

In lots of cases, your prescriptions are covered under AmeriHealth 65 NJ HMO, AmeriHealth Rx PDP if they are filled at a network pharmacy or through our mail-order pharmacy service. There are thousands of network pharmacies, including:
- national chain and independent retail pharmacies
- long-term care and home-infusion pharmacies
- Indian Health Service/Tribal/Urban Indian Health (I/T/U) Program pharmacies
